Who are the Professionals sent to your home?

  Registered Nurses (RN's) - providing individual care and case management for medically prescribed treatments.

  Licensed Vocational Nurses (LVNs) - working under a physician's order and RN supervision administer medication and treatments, and assists the patient with returning to independent activities of daily living.

  Certified Home Health Aids - provide personal care and/or light housekeeping.

  Physical Therapists, Occupational Therapists and Speech Therapists - provide rehabilitative services in order to assist patient's return to higher levels of functioning. 

  Medical Social Services access community resources to acquire the assistance patients need.

  Dieticians assist the patient's to follow and plan the proper diet ordered by the physician.
